These PDF forms require Adobe Reader version 8 or higher to work properly. (Adobe Reader is free software that you can download from the Internet.)

Web Registration is now available.

If you submit a form via the email submit button, and you do NOT get email confirmation back that your registration was received, YOU ARE NOT REGISTERED. Please contact us at: (905) 684-6821 if you have any questions.

These forms will not be available after July 19th. You must call after this date.

To register “online” download the appropriate form.

Do one of two things: (The first method is preferred as there may be issues using the second method, depending on your email and browser settings. Dialup email submission will not work.)
