Registration Procedure

Use the proper registration card.

Adult/Family Card (white) This is for adults age 21 and over, for children through grade 8 (entering grade 8 in September), and for married couples under 21. Single adults should each fill out their own card. Parents and young children should register on one card. Remember to list the names of teens you are sponsoring. A signature is required.

Young People’s Card (yellow). A separate card should be filled out for each person entering grade 9 in September, up through age 20.

• A sponsor of the same sex (at least 30 years old or married) must be provided for every person under 21 years of age. This information should be included on the yellow registration card. Contact your sponsor to make sure he/she agrees to accept this responsibility.
• Write your name on the registration card as desired on your name tag: i.e., Bill Jones or William Jones. A signature is mandatory and as a last resort will be obtained at registration.
• Print clearly or type, and be sure each card is completely filled out.

  Proper postage for mail to Canada is required. Check with your local post office.
